FeNO Testing

Why does FeNO matter?

Despite advances in treatment, asthma continues to place a significant burden on both patients and healthcare systems.1 This article explores how FeNO can be utilised as an effective and accurate biomarker for asthma diagnosis, management and predictor of risk.2-4

It is estimated that up to 73% of asthma cases may be underdiagnosed, while 33% of patients diagnosed with asthma may not even have the condition at all.10

Asthma is the 4th most common reason for adult hospitalisations and being responsible for around 1,000 deaths every day.5,6 Two-thirds of which could be avoided.7

FeNO testing is a non-invasive and easy tool which measures the level of nitric oxide in a patient’s exhaled breath.2,4 Each test produces a single FeNO score, which reflects the individual level of airway inflammation present in each patient.8,9 Elevated FeNO levels serve as a biomarker for eosinophilic airway inflammation, a key characteristic of asthma, and are associated with approximately three times greater risk of potentially severe asthma exacerbations.6,8,9

How can FeNO help improve asthma management?

Symptoms of asthma can often fluctuate in both timing and intensity, making diagnosis and treatment challenging.4 It is estimated that up to 73% of asthma cases may be underdiagnosed, while 33% of patients diagnosed with asthma may not even have the condition at all.10 As a result, symptoms alone are not always predictive of future risk, instead objective measures such as FeNO are essential for supporting the diagnosis and management of asthma.4

FeNO testing can help add clarity and confidence to asthma management.11 Recommended by many international and national asthma guidelines, such as NICE, ATS, and ERS, FeNO is highlighted as a key instrument in asthma care.12-15 With the NICE guidelines even noting it as a front-line diagnostic tool for asthma in both adults and children.12

There is no one-size fits all approach with asthma management.16 In a clinical practice, FeNO not only helps increase diagnostic accuracy, but can also be used to assess whether inhaled corticosteroids (ICS) treatments are likely to be effective.2,4 This allows for more personalised treatment plans by identifying patients who are likely to respond to ICS and monitoring their ongoing adherence and inflammation levels.2,4

Through integrating FeNO into regular reviews, it can be used to identify patients who non-adherent to their medication, practising incorrect inhaler technique and prevent patients from receiving ICS inadvertently or higher than necessary doses.2,17 FeNO-guided asthma management helps to optimise treatment and has been shown to improve patient outcomes and reduce exacerbations by up to 50%.4,17-19 Beyond strengthening patient care, this approach can also help reduce costs by optimising medication use, minimising exacerbation risk and subsequent hospital admissions.4,17,20
